How often do I need to come in?
It depends on your condition and how well it's controlled. Most chronic care patients visit every 3–6 months. We'll build a schedule that fits your needs.
Can you manage more than one chronic condition at once?
Yes. Many of our patients have multiple conditions; we coordinate everything in one place so you're not juggling multiple providers.
Will I see the same provider each visit?
Yes. Continuity is central to chronic care. You'll see the same provider so they know your history and your goals.
Do I need a referral to come for chronic care?
